Narrative

Employee was walking down a set of steps while maintaining 3 points of contact. The miner missed the last step in the set and fell forward striking his side on a hand rail. The miner suffered a fracture to one of his ribs. The following were not a factor: rules, regs, mining equipment, skills, training, proficiency, PPE, or safety devices on equipment.
