## Narrative

Employee says he injured himself on 7/21/15 while attempting to lift a gear box.  He said he continued to work ""thinking nothing of it."" He did not notify us until a week later. Doctor diagnosed him with a strain and performed no treatment, but asked that he return in a week.  Last doctor's report on 7/29/15 showed he may have a possible strain.
