## Narrative

On 5/4/2015 employee reported to his supervisor that he bumped his elbow. On 7/28/2015 employee was diagnosed with Tennis Elbow (lateral epicondylitis). The employee has 8 years of repetitive motion of the elbow performing activities such as swinging a hammer, shoveling, pushing and pulling levers, pulling cables, etc.  Employee was given work restriction on 7/28/15.
