## Narrative

While removing a pulley from a jaw crusher pulley shaft suspended by forklift chain 4-5"" off the ground in the maintenance bay the shaft came free and hit the tip of an employee's left index finger, breaking the bone. The supervisor had instructed the employee to hold onto the outside of the pulley, but at some point the employee put his finger inside the pulley where it was hit.
