Narrative

Employee was wet dusting a punch cut between #6 & #7 entries. His scoop was parked on the inby corner of the #7 intersection, he attempted to reenter the scoop on the rib side, a piece of loose rib rolled off the corner, striking his back and pushing his body into the scoop.
