Narrative

While moving a section of 6"" aluminum water line, the pipe rolled and the tip of his left index finger got caught between the pipe and a section of belt structure. The accident happened because the employee had failed to secure the pipe. The ee had been properly trained and did not violate any rules or regulations. The ee has a good safety attitude.
