## Narrative

While testing a water hose (for cutting purposes) at 46 blk Main West when a 6"" ball valve failed and struck him in the left knee. Initial x-rays were negative, employee had next two weeks off, however a later MRI (8/27/15) showed a hairline fracture to the proximal end of his left tibia. No problems with mining methods, ppe, training or compliance issues.
