An air exam, dust from a screw blew directly onto a monitor causing an alarm and NO reading of 141@4ppm. The dust was removed, the alarm cleared, and air readings returned to normal. This form is submitted to terminate Citation No.8820578 and should not be construed as an admission that the matters described herein constitute a reportable accident, injury, or illness.
