## Narrative

On 8/11 IE was running a D-10 on the plant coal pile when the dozer lurched. This lurch caused the IE to have neck and back pain. The IE was evaluated by a physician the same day and was released to return to work. The IE has continued to work over the past month, but at his appointment on 09/11 he was not released to return to work due to pain in his neck and back.
