## Narrative

 a fully trained miner was putting fire suppression pipe together at the take up . The employee was bent over facing the rib and a piece of stone 6""x2""x1"" fell from the rib striking him in the neck and shoulder area. The employee never lost consciousness and PPE was not a factor.
