## Narrative

The injured miner was bending an 8 foot roof bolt. When he began the bend, he felt a burning sensation in his left shoulder area. The following were not a factor: rules, regs, proficiency, skills, equipment, protective devices, clothing and attitude. The miner did not begin to lose time until the date of 9/18/15.
