Narrative

Employee was operating a continuous miner (CM) and he was loading the coal transporter, a piece of loose roof fell and landed on top of the CM. He trammed the CM out from unsupported roof and using a 5' pry bar, he attempted to dislodge the material. The material shifted and slid off the side of the CM striking his leg. He began missing time on 9-17-2015.
