Narrative

Employee notified safety of long term shoulder pain on 1/14/15. He was diagnosed with degenerative shoulder pathology and had surgery on 8/11/15. Surgical notes received on 10/8/15 noting AC joint arthritis, partial thickness articular surface tearing of the subscapularis tendon and diffuse labral tearing. All conditions of cumulative trauma from LW maintenance/utility activities.
