Narrative

The employee was carrying a bag of rock dust on his shoulder when he stepped on an uneven section of mine floor. This caused him to fall and injure his knee. The following were not a factor: rules, regulations, mining equipment or systems, protective devises, equipment, clothing, skills, proficiency or training. Attitude may have been a factor.
