Narrative

On 10/2/15, EE was carrying a valve & lost his balance. When he lost his balance, he contacted a handrail & valve he was carrying contacted his side/rib area (left side) resulting in a contusion. Took him to ER, x-rays were negative. Full Release issued. On 10/7/15, he said he ""re-aggravated"" it & he went to doctor. Restrictions were issued. (10/9/15-1st shift missed)
