Narrative

On 10/9/15 a roof fall was discovered in an air leg that is in common with the Main West Belt between 29.75xc-30xc. The fall is 70 feet in length, 17 feet in width and is approximately 9 feet in height. The area was initially bolted with 8' point anchor resin assisted roof bolts. Weathering of the roof was determined to be the cause of the accident.
