## Narrative

Employee was working alongside two other employee's replacing a gear box on a head pulley that fed into the hopper of the tail section of the next conveyor, in which employee was standing. Employee when working became disoriented, circled in place and fell over, falling off the area he was standing and landing on his head and neck.
