Narrative

The employee was attempting to climb the ladder in to a 777 truck at the start of the shift. He fell backwards while trying to get on the first step and injured his back. The following were not a factor: rules, regulations, mining equipment or systems, protective devises, equipment, clothing, skills, proficiency or training. Attitude may have been a factor.
