Roof fall was discovered in a four way intersection of entry #4, xc 41 on the main west #2 travelway. The area had been mined in August, 2001. The roof was supported by fully grouted resin bolts ranging from 4 to 8 feet. No injuries resulted from this occurrence. The fall measured 20'X20'X10' high and did not reveal any obvious condition that may have contributed to failure.
