## Narrative

IE stated he was installing a roof bolt and it bent. He lowered the drill pot and started retracting the boom. IE was retracting the boom, he had his hand lying over the lip of the tray. During this time, one of the roof bolts located in the tray slid forward, crushing his right middle finger between the bolt and the lip of the tray, causing a laceration and a fracture.
