On 11/19/15, Employee self-procured medical treatment and was prescribed an antibiotic and pain medication for a L-eye corneal abrasion. While he was lying on his back looking up to clean dirt from bolts, dirt fell into his L-eye and caused irritation. He was released to full duty on 11/20/15 by the company assigned physician.
