## Narrative

While carrying a pump, the employee slipped and stumbled and afterwards complained of pain in his right lower abdomen.  The mine's Safety Department received notification on Dec 18, 2015 that this is a hernia and the employee will undergo surgery on Dec 29, 2015. The incident occurred because the employee was not aware of the bottom conditions.
