Narrative

The employee was carrying a bundle of 5 roof bolts to supply the bolting machine. When he lifted the bolts to place them in the tray, he felt a pain in his left shoulder. The following were not a factor: rules, regulations, mining equipment or systems, protective devices, clothing, skills, proficiency or training. Attitude may have been a factor.
