Narrative

100 lb. barite sack rolled onto shoulder of injured temporary staffing employee. Diagnosis of other sprain of left shoulder joint - minor and Pain in left shoulder - minor. Doctor released with no restrictions on 12/11/15. Sore shoulder prompts doctor's visit on 12/17/15 resulting in ""Medium Work"" restriction and left shoulder/scapular contusion diagnosis.
