Narrative

The employee was struck by a section of coal rib when EE squatted down in front of it. The incident occurred because the employee did not view this rib as a potential issue. The employee was properly trained and has a good safety attitude. The incident did not involve any mining equipment or any aspect of the mining system. EE had EE's proper PPE.
