## Narrative

EE was standing on a belt to help set timbers during roller service. Group did not follow company policy by confirming with designated person that they were standing on the belt. Belt was turned on and EE was knocked down injuring back. The following were not a factor: regs, skills, proficiency, training or protective devices. Rules and attitude were a factor.
