Narrative

Employee was walking down steps that were installed to prevent falls on bank when they slipped/tripped and fell injuring right hip. There were no compliance issues. It didn't involve job skills proficiency training or attitude PPE or protective devices no equipment involved. No other part 50 criteria applies.
