Narrative

Roof fall occurred on the Old Main North Track inby the Dust Hole Switch. The fall is 40' in length X 16' in width and 14-16 feet in height. The area was initially bolted with 8' point anchor resin assisted roof bolts. Weathering of the roof was determined to be the cause of the accident.
