Narrative

On 1/12/2016, an employee reported pain in lower back throughout the shift. On 1/13/2016, the employee was evaluated, the employee was on scheduled days off until 1/21/2016 at which time EE was re-evaluated and given restrictions that would prevent EE from performing normal job.
