## Narrative

The employee was struck in upper lip while changing a replacement belt roller.  The accident occurred because the employee was not paying attention to the task at hand.  The employee was wearing the appropriate PPE and was properly trained in employee's job function.  The equipment involved was a bottom belt roller associated with a conveyor belt system.
