Unplanned roof fall discovered reportable at 138 block #7 entry of 9 South. Fall approx. 50' long, 12' high and unknown width (traveling toward return stopping). Area of weathered loose clay discovered at, and above, anchorage zone, with cutter running outby in the entry toward 137 blk. Improper bolt installation may have also been a contributing factor.
