Narrative

Employee was making pre-shift examination when ee stepped out of vehicle at the 11WHG and stepped on a piece of concrete block & fractured foot and ankle. Cause of injury was stepping on uneven floor. Incident didn't involve any aspect of compliance with rules, regulations, & PPE. Miner proficiency may have been a factor. Started missing work 3/12/2016.
