## Narrative

EE stated EE was installing the 1st rib bolt on the opr side. After drilling hole, EE inserted top of the bolt into hole and the plate became stuck in gob. EE jerked the bolt & felt a pain in EE's left shoulder. EE was transported outside & refused ambulance. At Bluefield Reg. Hosp ER EE rec'd X ray. EE was restricted from work until visit to specialist. Started missing on 3/17/16.
