Narrative

EE was operating #328 shuttle car on the 16 Headgate Section. EE was going to get out of the shuttle car to hang up a piece of miner cable. In the process of EE walking to the cable a piece of rock that measured 6'X3'X8"" fell from mine roof & struck EE in the head, shoulder, & forearm. This caused EE's head to come in contact w/shuttle car & for EE to fall on EE's back.
