## Narrative

Employee was standing near 10-wheeler boom truck about 12-15"" from hoist cables at #2 D/L. Two other employees were using a 8 lb. sledge hammer and RR maul to drive wedge out of hoist socket. A piece of metal sheared from the maul and traveled approx. 12-15"" striking employee in back of right calf.
