On the above date this employee was using a 1"" impact gun and felt a sharp pain in upper left arm/shoulder. He has been treated for first aid since then and was out for personal illness for almost a year. Only now receiving any treatment for work injury. He had exploratory surgery and first day of lost time is 03/24/2016. The validity of this injury is in question.
