Employee struck head on a roof bolt sticking out of the mine roof. EE completed an incident report and never sought medical attention until 2/25/16. EE was given a full release to return to work which EE did and continued to work until 3/28/16 at which time EE was taken off work due to spinal stenosis which is narrowing of the spinal canal and flattening of the cord.
