## Narrative

Employee was putting a fork onto a Lo Trac and was going to slide the fork closer to the adjacent fork which was already in place.  The fork was not completely in place on the slide bracket and when EE attempted to move it, the fork fell.  The fork landed on the tip of EE's finger and mashed it against the mine floor.
