Narrative

The mine was notified on 4/6/2016 that this former EE (retired) has been granted an additional PPD of 5% for Occupational Pneumoconiosis. EE's total PPD is 10%. EE's date of last exposure was 12-27-2014 and EE's filing date was 9-15-2015. This incident was caused by the employee's failure to wear respiratory protection. EE did not violate any rules, regulations or policies.
