Notification was received on 4/6/2016 that this former employee (retired) was granted a PPD of 20% for Occupational Pneumoconiosis. The date of last exposure was 7-31-2014. The date of filing was 9-21-2015. This incident was caused by the employee's failure to wear respiratory protection. Employee did not violate any rules, regulations or policies.
