Narrative

While waiting for off side operator to finish putting up inside bolt injured employee was reaching across the top of the inch tram levers on the bolter to get some roof bolt plates. While reaching over, a piece of draw rock measuring 81"" x 42"" and 0-3"" in thickness fell, driving employee into bolt. EE's lip was cut from the impact of hitting the guard over the inch tram levers.
