Narrative

While working on the tracks at the South West Mine for Leach Radial Stacker #3, an employee was standing back at approximately 111 feet away from the track. While watching the track make a move ee felt something hit face. EE walk over to a co-worker and ee was bleeding. EE was taken to the clinic and a piece of fly metal was found lodged in upper lip.
