Narrative

Employee had ongoing shoulder pain and had surgery on 2/22/16 for significant synovitis and disease of the biceps tendon, AC joint degeneration repair, side-to-side rupture of the supraspinatus repair and small focal posterior labral tear. Surgical notes received on 4/7/16 where the injury was determined to be work related from cumulative trauma.
