## Narrative

Employee informed us of increased pain since closing a valve underground but continued to work until surgery on 2/8/16. MRI was performed and subsequent surgery to repair tendons and ""clean out"" shoulder. Diagnosis was tendon tearing, bursitis, and mild degenerative arthropathy. Medical notes received on 4/7/16 and determined this was work related.
