Narrative

The mine was notified on 4/11/2016 that this retired employee has been granted an award for Occupational Pneumoconiosis. EE's PPD is 10%. EE's date of last exposure was 12/29/2014 and EE's filing date was 9/15/2015. This incident was caused by the employee's failure to wear respiratory protection. EE did not violate any rules, regulations or policies.
