Narrative

Employee was removing worn paddles from material washer by lifting paddles to edge of tub and tipping them over the edge. Employee lifted paddle to edge and didn't clear lip of tub causing the paddle(40lbs) to fall approx. 10in. smashing employees right middle finger. Resulting in fracture to tip of right middle finger. Management notified of incident on 4/25/2016 @ 8:30am.
