## Narrative

Employee was using an extension pole to unhang the walk-through bolter's cable rom the cable hooks attached to the mine roof.  During the process, EE lost their footing on uneven mine floor, causing EE trip and stain left shoulder.  EE began missing time on 5-2-16.
