## Narrative

Discovered unplanned roof fall 23W 2 block 1A to 1 crosscut and part of #1 intersection. Fall was 35' long, 16' wide and 12' high. Unbeknownst to management, the installed bolts did not anchor into competent strata. No problems with mining machines, training or PPE. Citation was issued for not following roof control plan. Scoop charger partially covered up.
