Narrative

EE states was performing inspection & filling out JHAs week of 4-3-16. EE was standing on uneven work area. Arch of foot was on a hose fitting. EE continued working & noticed soreness over next few weeks. Finally, 5-2-16, EE noticed foot was swollen & went to Urgent Care. EE said nothing of the injury between 4-2-16 & 5-2-16. Validity being questioned & under review at this time.
