Narrative

While dragging waterline, the employee slipped and fell landing on right side. The employee complained of pain in back. The accident occurred because the employee failed to pay attention to the conditions of the mine floor. Employee had on appropriate PPE and has a good attitude. Employee was properly trained on the task.
